Objetivos del proyecto
Chronic diseases are becoming more prevalent and it is common to find patients with more than one. Its approach requires strategies aimed at empowering and endowing the patient with competencies that favor the self-management of their own diseases. The main objective of the Responsible Care Program, implemented in the SCS within the Chronicity Strategy of Cantabria, is to provide people with chronic diseases and their caregivers with tools to help them better manage their health and, together with healthcare professionals, achieve better management of your disease. This program seeks to move towards responsible care by expanding the knowledge and skills of people with chronic diseases, improving the relationship between them and the health system and all this, through interaction and support among equals. The main objective of this project is to study the impact of the Responsible Care program on people with chronic diseases and their carers in the self-management of their chronic diseases. The patient endowed with tools of self-management of his chronic disease will have greater knowledge and better control over his chronic disease, a greater adherence to treatment, greater perception of quality of life and will consume less health resources derived from the complications of his illnesses.
